Cascade error on agentic task — Plan, Run Code, and Tools Agent all failing in sequence

Description
User ygkang4354 submitted a screenshot on 19/05/2026 showing a full cascade of agent step failures with no accompanying description. The screenshot shows the following steps all marked with error: Plan, Run Code (×6), Tools Agent (×2). The app displayed a generic error message: "Something went wrong. Please try again. This error has been reported."
No steps to reproduce were provided. Team requested reproduction steps on 21/05/2026 — no response from user as of 25/05/2026 despite a follow-up ping. Root cause unknown without logs or repro steps.
Screenshot attached (see attachments). The pattern of repeated Run Code and Tools Agent failures suggests either a retry loop hitting a persistent error, or an upstream model/tool dispatch failure causing the entire agent chain to collapse.
